Overview
Do not delay resuscitation to assign an APGAR score. Begin NRP interventions based on assessment of breathing, heart rate, and tone regardless of score.
The APGAR score is a quick assessment performed at 1 and 5 minutes after birth to evaluate the newborn's condition and need for intervention.
- The 1-minute score reflects the need for immediate intervention
- The 5-minute score better correlates with long-term outcomes
- If the 5-minute score is < 7, continue scoring every 5 minutes up to 20 minutes
- APGAR should never be used alone to diagnose birth asphyxia
The 5 criteria are not of equal significance. The original article noted that HR and respiratory effort were most important, while color was least important. To improve ease and utility, the criteria were not weighted for item importance.
